Tuberculosis Pathogenesis and Drug Resistance
Mtb infects alveolar macrophages in the lungs, initiating disease. Granulomas form to contain infection, with transitions between active disease (bacterial spread, impaired immunity) and latent infection (controlled growth). Reactivation occurs with immune suppression. Drug resistance arises via plasmid-encoded enzymes, efflux pumps, porin and PBP mutations, and metabolic pathway changes enabling survival under antibiotic pressure.
